Urgent but Simple Ad Design - Upwork

We need to create a simple ad that is 2 inches tall and 1.5 inches wide that incorporates part of the text and the colors of one of our existing ads (which has different dimensions) along with our QR code. It should be extremely quick for someone with graphic design experience (less than an hour). Specifically, from the Utah Advisory Council Ad, we need our name ("YouthPLAYS") and "Plays for production, competition and the classroom." We'd like our URL ("www.youthplays.com"), but instead of the space currently taken up by the other text in the ad, we'd like to substitute our QR code.
